4.1.11 Establish and improve workers’ occupational health monitoring files
According to regulations, employers Units should establish occupational health surveillance files for workers (including temporary workers) who have labor relations.
The labor roster should be established and archived separately before taking up the job, while on the job and after leaving the job.
Occupational health monitoring files should include the following contents:
—General profile of the worker such as name, gender, age, place of origin, marriage, degree of philanthropy, hobbies, etc.;
—Worker’s occupational history, past medical history and exposure history to occupational disease hazards;
—Results of monitoring of occupational disease hazard factors in the corresponding workplace;
—Occupational health inspection results and handling;

—Worker health information such as occupational disease diagnosis and treatment.
