The first part of the third page

1 is the purpose of choosing to go to Australia (1 temporary residence, 2 permanent residence, 3 both).

Information of children going to Australia (surname, first name, date of birth, country of passport issuance, passport number), A is the information of one child, two children go, the information of the second child is written on B, and so on.

Part B of Page 4 This page is to be filled out by parents who are not going to Australia.

Fill in the unaccompanied parents' information (surname, first name, address and postcode, landline, mobile phone, relationship with children, passport number or ID number or driver's license number, signature (in Chinese) and date). If both parents don't go, write their parents' information in 3 and 4 respectively. If only one parent doesn't go, write the information of the parents in 3.

Note: Write only Arabic numerals (1, 2, 3 ...) to get the date, postal code and telephone number. The final signature is in Chinese, and the rest is in English.
