How to write an application for collecting contraband at the station?

Write the title, receive the document, handle the request, explain the reason and sign.

1. Title: Write "Disposal Application" at the top of the paper, and leave a blank line at the bottom, and write down the name, company name and date.

2. Receipt: In the text, the first paragraph is written as "receipt", and the name, quantity and source of confiscated contraband are listed below.

3. Disposal request: Write "Disposal request" in the second paragraph, and explain the contents of the disposal request in detail below.

4. Reason explanation: Write "Reason explanation" in the third paragraph, and explain the reason of your disposal request in detail below.

5. Signature: Leave enough space at the bottom of the application form, and write your signature and date on the last line.
