First, the employer questionnaire

(1) Fill in the requirements

A) The employer questionnaire is a form filled in on the spot during the employer survey, and each employer fills in one.

(2) Filling method

A) Name of the employer: fill in the full name of the rural land employer, starting with the name of the township where the employer is located, and fill in the name of the township (group) collective economic organization.

B) employer code: fill in the employer code compiled according to NY/T 2538.

C) Name/contact number of the person in charge of the employer: fill in the name, contact number and/or mobile phone number of the current person in charge of the employer.

D) Address/postal code of the person in charge of the employer: fill in the mailing address and corresponding postal code of the person in charge of the employer.

E) Certificate type/certificate number of the person in charge of the employer: select the certificate type of the person in charge, tick "√" before the corresponding certificate type, and fill in the certificate number of the corresponding certificate type. If "other certificate types" are selected, the specific name of the certificate type shall be indicated, and the certificate number of the corresponding certificate type shall be filled in.

F) Investigation notes/investigators: record the matters that need to be explained in the investigation of the employer, including: 1) the change of the employer and its responsible person; 2) Rural land ownership [state-owned, collective-owned (owned by village collective economic organizations and owned by villagers' groups)]; 3) Rural land contracting; 4) Other circumstances that need to be explained. All investigators shall sign or seal for confirmation.

G) Review opinion/reviewer: the reviewer will comprehensively review the survey results, and if there is no problem, fill in "qualified". If problems are found, fill in "unqualified", indicate the errors and put forward suggestions for treatment. The auditor shall sign or seal for confirmation.

H) Date: Fill in the date of investigation in Arabic numerals and fill in the complete year.

Second, the contractor questionnaire

(1) Fill in the requirements

A) The contractor questionnaire is a form filled in by the contractor on site during the investigation, and each contractor shall fill in one copy.

D) The contractor questionnaire is divided into five parts: basic information of the contractor, contract/warrant information, information of farmers' family members, survey notes and audit opinions, which are separated by thick solid lines.

F) The information of the contractor's representative must be filled in the first line of the list of family members.

(2) Fill in the explanation

A) Employer Code: Fill in the complete code of the employer.

B) Contractor code (abbreviated code): fill in the abbreviated code of the contractor code (the sequential code part in the contractor code).

C) Contractor (representative): fill in the name of the contractor's representative (household contracting) or the name and position of the contractor (other contracting methods).

D) Contact telephone number: fill in the contact telephone number and/or mobile phone number of the contractor (representative). If the unit is a contractor, fill in the contact telephone number and/or mobile phone number of the legal representative of the unit.

E) Contractor's address/postal code: fill in the address of the contractor's homestead or other long-term residence, starting with the name of the township (town), and fill in the specific name of the village, group and house number. If the contractor contracted by other means is a unit, fill in the common office address or registered address of the unit. Fill in the postal code corresponding to the contractor's address.

F) Certificate type/certificate number of contractor (representative): Select the certificate type of contractor (representative) according to the actual situation, tick "√" before the corresponding certificate type, and fill in the certificate number of the corresponding certificate type. If "Other" certificate type is selected, the specific name of the certificate type shall be indicated and the certificate number of the corresponding certificate type shall be filled in.

G) Whether there is a contract/contract number: explain the rural land contract according to the actual situation, and tick "√" before the corresponding options. When "Yes" is selected, fill in the corresponding contract number; When "None" is selected, the number column is filled with "/"symbol.

H) Whether there is a business license/business license number: explain the land contract business license according to the actual situation, and tick "√" before the corresponding options. J) Acquisition (contracting) method: select the way for the contractor to acquire the contracted management right of rural land, and tick "√" before the corresponding option. When "Other" mode is selected, specify the specific acquisition method (signing).

K) Total number of family members: fill in the total number of farmers' family members.

L) Member's name: fill in the names of family members, with the head of household as the first place.

M) gender: fill in the gender of the corresponding family members, including "male" and "female"

N) Relationship with the head of household: fill in the relationship between family members and the head of household, including the head of household, spouse, husband, wife, son, adopted son or stepson, son-in-law, female, adopted daughter or stepdaughter, daughter-in-law, grandson, granddaughter, grandson, granddaughter and granddaughter. S) Review opinions/reviewers: the reviewers will comprehensively review the survey results. If there are no questions, fill in "qualified"; If problems are found, fill in "unqualified", indicate the errors and put forward suggestions for treatment. The auditor shall sign or seal for confirmation.

T) Date: Fill in the date of investigation in Arabic numerals, and fill in the whole year.

Third, the contracted land questionnaire

(1) Fill in the requirements

A) The questionnaire on contracted plots is a form filled in on the spot during the investigation of contracted plots, which is filled in by contracted farmers, and the number of forms is determined according to the actual number of plots. When additional tables of more than three plots are added, the titles "Owner Code" and "Contractor (Representative)", "Investigation Description" and "Audit Opinion" shall be filled in each table.

E) The questionnaire of contracted land parcel is divided into six parts: header information, basic information of land parcel, landmark information, boundary signature information, survey notes and single-core opinions. All parts except the header are separated by thick solid lines.

F) The questionnaire of contracted plots can be added by itself according to the number of plots, and the header and footer format must be kept.

G) When the number of boundary points in the local block is greater than 6, write the control point information of 1, 2, 3, 4, 5 and the last point.

(2) Fill in the explanation

A) Employer code: fill in the completed employer code.

B) Contractor (representative): fill in the name of the contractor's representative (household contracting) or the name and position of the contractor (other contracting methods). Fill in the name (or name) of the actual cultivator (or unit) in the non-contracted land, and indicate the contractor code if there is a duplicate name.

C) Sheet number: Fill in the sheet number of the survey base map according to the coding rules determined by "framing and numbering" in "Mathematical Basis and Precision Index" in the General Terms and Conditions.

D) Land name: fill in the small place name, customary place name or common name of the land under investigation and express it concisely in local customary way.

E) Drawing code (abbreviated code): Fill in the abbreviated code (part of the sequence code in the drawing code) according to the rules of drawing code in NY/T 2538.

F) Contracted area: the area per mu recorded in the contract (or contract management certificate). The decimal places are consistent with the records in the contract (or the contract management right certificate). For non-contracted plots, the corresponding contracted area shall be recorded according to the actual cultivated area.

G) East/South/West/North: fill in the name of the corresponding rural land owner or feature.

H) Land use: fill in the planting, forestry, animal husbandry, fishery and other types, and tick the corresponding types. When "Other" is selected, fill in the specific purpose afterwards.

I) Land use type: fill in paddy field, dry land, irrigated land and other agricultural land types, and tick "√" on the corresponding types. When "Other" is selected, fill in the specific purpose afterwards.

J) Fertility grade: Fill in the fertility grade of cultivated land according to NY/T 1634, NY/T 309 or the local actual situation at the time of land grant.

K) Basic farmland: tick "√" for "Yes" or "No". If it is impossible to determine whether the plot is basic farmland, it should be indicated in the "investigation column" and filled in the corresponding column after being determined by the competent agricultural department in conjunction with the land department.

L) Boundary point number: fill in the boundary point number according to the method of "Boundary point number" in the survey of the contracted land, and the first and second boundary points will be closed to form the contracted land.

M) Landmark type: according to the actual buried landmark type, mark "√" in the corresponding position, and fill in the blanks for the types not listed in the table. If no landmark is set, you can fill in "None" in the blank and tick "√" in the corresponding position.

N) Category of the boundary line: according to the features and landforms actually attached to the boundary line, draw "√" in the corresponding position, and add "√" in the blank space for categories not listed in the table.

O) Location of the boundary line: according to the subject matter of the boundary line, draw √ on the outside, √ in the middle and √ on the inside.

P) Description of the boundary line: description of the nature, length, direction and trend of the boundary line, as well as other information that plays a key role in the description of the boundary line, such as "No.3 boundary point is far from the southwest corner of the house.
