Must obtain 14 weeks and above artificial termination of pregnancy surgery approval form. If a woman who is pregnant for 14 weeks or more does not meet the statutory fertility conditions (unplanned pregnancy) or meets the statutory fertility conditions, and requires artificial termination of pregnancy due to divorce or widowhood, the applicant must go to the township family planning office to receive and fill out the Approval Form for Artificial Termination of Pregnancy for 14 weeks or more (see annex), and submit the application form to the township family planning office.
