Take a foreign invitation letter or I-20 form (one for each) to the Personnel and Education Department of the hospital to receive the Approval Form for Persons Going Abroad on Business (or download it directly from the website of the Education Department), and return it after the relevant department heads agree to sign it.

2. Go to the Exit-Entry Administration Office of the Exit-Entry Municipal Public Security Bureau to receive the Examination and Approval Form for China Citizens Going Abroad.

3. Go through the formalities of leave, resignation and termination of employment contract.

Go to the Personnel and Education Department to check and stamp, pay the handling fee, and then go to the Exit-Entry Administration Office of Dalian Public Security Bureau to apply for a passport.

5. Apply for a visa in embassies and consulates in China, the relevant country.

6. If notarization is required, please go to the notary office to receive the notarial certificate and stamp it at the Personnel and Education Department.
