15, certification materials related to the factory inspection conditions of enterprise food.
(1) The self-inspection shall be submitted:
A) List of instruments and equipment used for inspection;
B) A copy of the legal right to use the main inspection instruments and equipment (such as purchase invoices or the right to use inspection equipment);
C) Copy of metrological verification (approval) certificate of inspection instruments and equipment within the validity period;
D) Two or more food inspectors (if employed, an employment contract shall also be issued; Copy of inspector's certificate with a contract period of not less than 4 years.
(2) Entrusting inspection Entrusting inspection (unless the inspection is prohibited by state regulations), the following certification materials shall be submitted:
A) Sign the ex-factory inspection contract (agreement) with the inspection and testing institution with qualification certificate (the agreement period of the ex-factory inspection entrustment contract is not less than 4 years; The inspection ability can cover the varieties that the enterprise applies for license and meet all the technical requirements required for ex-factory inspection as stipulated in product standards);
B) Copies of relevant parts of the qualification certificate (metrological certification) of the entrusted party and its attachments (i.e. list of inspection and test capabilities);
16. For the production (trial production) of the packaging materials (especially the inner packaging materials that are in direct contact with food) and food additives, the production license of the factory enterprise (including the original, copy and supplementary pages) and the factory inspection report of this batch of products (if the factory inspection report cannot be provided, the qualified packaging material inspection report issued by a third-party food inspection agency) and the factory certificate of the unpacked raw and auxiliary materials shall be provided.
17. If relevant laws and regulations and national industrial policies have other requirements for food production, other relevant certification materials (photocopies) shall be submitted;
18. Has the enterprise obtained the certification related to the quality and safety of food production (such as quality management system certification, food safety management system certification, etc.)? ), which can be provided at the same time.
Each of the above materials should be stamped with the official seal of the enterprise (if the food production enterprise to be established has no official seal, the applicant should sign and confirm by fingerprint; If it is a copy of the certificate or certificate, the words "the content is consistent with the original" should be written in the blank space of each page, and the official seal of the enterprise or fingerprint should be affixed to confirm the signature), and the above-mentioned complete application materials should be submitted (unless the application form is provided separately); Other materials are bound into a book) in triplicate.
All the materials and contents submitted above are true, legal and effective; If it is a certificate or voucher, the original should be presented during the on-site inspection. When receiving the certificate at the government service window, except in accordance with the unified requirements of the government service window, the original food production license (including the original, copy and supplementary page) shall be returned in full (if the original certificate text cannot be returned in full, it shall be submitted to a newspaper at or above the provincial level with the corresponding accurate content "declared invalid"), and the government service window can only give a new certificate.
