On the third page of the manual, when filling in the registration form of internship time in each subject, it should be noted that the time and weeks filled in must be consistent with those filled in the following pages. (This form may be filled in in advance, but some hospitals will temporarily change the internship department and time during the internship rotation, so the time and number of weeks filled in may be different. Make a copy of the registration form of internship time in each subject in advance, and then formally fill in the form after the internship, or fill in the columns after each internship. )

6. Manual: internship registration form and internship assessment form, in which the internship time and weeks must be consistent with the internship time registration form of previous subjects.

7. Manual: The total evaluation table and weeks of student internship assessment must also be consistent with the previous one. The number of rotation weeks, scores of various subjects and standardized scores must be filled in.

8. The last pages of the manual, clinical practice certificate. The last page must be filled in, because the certificate must be torn off and kept by yourself in the future.

9. After the internship, please make a complete copy of the manual and keep it for yourself.

10. When returning to school after the internship, be sure to bring two two-inch photos back to school and paste the graduate appraisal form and registration form.
