(1) Application Form

The items in the application form shall be truthfully filled in, and the same columns as the certificate shall be consistent with the contents of the certificate. Fill in the main points with form a.

The certificate number is filled in the upper right corner of the certificate.

For China Ice certificate number, the letter "I" must be filled in, and the rest can refer to the arrangement of certificate numbers in Table A.. For example: I14470zc21100321.

Third, fill in the certificate.

The first column:

Fill in the full legal name, address and country name of the exporter.

The exporter must be a company registered in our bureau, and the address should be clear to the street or road name.

The second column:

Fill in the legal full name, detailed address and country name of the consignee, and the address must be detailed to the street. When the final consignee is not clear, you can fill in "According to the order" in this column.

The third column:

Fill in the departure date, car number, loading port and unloading port.

If the number of means of transport is ominous, you can simply type in the mode of transport, such as "sea" or "air".

The unloading port must be an Icelandic port.

The fourth column:

Fill in what needs attention in the remarks column.

The fifth column:

Fill in the article numbers in sequence, with a maximum of 20 items. If the document number exceeds 20, you have to apply for another certificate.

The sixth column:

Fill in the mark and number on the parcel.

There must be no words made abroad on the shipping mark. Do not fill in "See Bill of Lading" or "See Invoice". The mark is in the form of a pattern and can be pasted. Copy and paste the shipping marks in the blanks from the fifth column to the eleventh column of the certificate, without leaving edges or covering them. If one page cannot be pasted, it can be pasted to the second page, and so on.

The seventh column:

List the detailed name of each kind of goods and make it correspond to the name of the goods on the invoice and the H.S. code of the certificate.

When filling in this column, you should pay attention to:

1, indicating the packing quantity and packing method. If the goods are unpacked, they should be marked as "bulk" or "naked". If the clothes marked on the shipping mark are hung, the total number of pieces (PCs) can be marked;

2, the number of items shall not exceed 20;

The eighth column:

The name of each item shall be marked with the corresponding six-digit Harmonized System Code (H.S. code);

Column 9:

Indicate the origin standard of the declared goods enjoying preferential treatment.

Completely made in China: fill in "WO"

Including raw materials from China and Iceland, produced by qualified raw materials: fill in "WP".

Production using non-original materials conforms to the provisions of product-specific rules: fill in "PSR"

The tenth column:

Fill in the unit of measurement for the quantity of goods. The unit of measurement should be based on the sales unit listed in the invoice. Such as "liter" and "cubic meter".

Fill in the actual invoice number and invoice date.

Fill in the date of declaration, and the applicant will sign and affix the official seal.

Fill in the date of signature, which shall be signed by the authorized visa officer, and the visa institution shall affix the seal of the visa institution and mark "Form A" for the record. If the certificate is issued later, print the words "retroactively issued" in column 12.

The certificate applied for before shipment of goods is a normal certificate; You can apply for a post-issue certificate within one year after the goods are shipped, and the application requirements are the same as those of the GSP post-issue certificate. At this time, the column 12 of the certificate is printed with the words "issued retroactively".

Certificate change must provide the original certificate, and the old and new certificates should be the same batch of goods. If the certificate is stolen, lost or damaged, the exporter can reissue the certificate. It is required to provide a copy of the original certificate or a second copy, as well as copies of relevant commercial documents, such as bills of lading and customs declarations. Except for the column of "exporter's declaration" and the column of "certificate of visa authority", the contents of each column of the reissued certificate shall be consistent with the original certificate; The issuing date of the reissued certificate is the actual issuing date; The application date for the replacement certificate is the actual application date; The words "original certificate of origin (No.:_ _ _ _ _ _) and approved true copy" will be automatically marked in the remarks column by the visa system.
