Fill in the full legal name, address and country name of the exporter.
The exporter must be a company registered in our bureau. When an exporter declares on behalf of another company, he can add "agent (O/B)", "forwarding (C/O)" or "VIA" after his name and address, and then mark the name (or address) of the company he represents. If the head office contains an overseas name and address, it shall not declare it.
The second column:
Fill in the legal full name, address and country name of the manufacturer.
If the certified products contain more than one manufacturer, they should all be listed. If the exporter wishes to keep the information confidential, he can fill in "Request from Authorized Organization". If the manufacturer and exporter are the same, "same" should be filled in.
If the production enterprise is different from the export enterprise, and the nature of the enterprise cannot be determined by the content of the production enterprise, the business license of the production enterprise shall be provided for future reference.
The third column:
Fill in the full legal name, address and country name of the consignee.
When the final consignee is not clear, you can fill in "According to the order" in this column.
The fourth column:
Fill in the departure date, car number, loading port and unloading port.
You can add "ON" or "ON/AFTER" before the departure date to indicate the exact shipment date or the possible shipment date in the next few days.
If the number of means of transport is ominous, you can simply type in the mode of transport, such as "sea" or "air".
The loading port must be a domestic port.
The unloading port must be a port in Peru.
The fifth column:
This column can indicate the customer order number, letter of credit number, etc.
If the invoice is issued by a non-contracting party operator, the legal registered names of the non-contracting party operator and the goods manufacturer should be filled in this column. The name and address of the manufacturer in this column should be the same as that in the second column.
The sixth column:
Fill in the article numbers in sequence, with a maximum of 20 items. If there are more than 20 items, you must apply for another certificate.
The seventh column:
Fill in the quantity and type of package. Fill in the detailed name of each kind of goods so that customs officials can identify them during inspection and make them correspond to the name on the invoice and the name on the H.S. code. If the goods are in bulk, it should be marked as "bulk".
The eighth column:
Indicate the six-digit Harmonized System code (H.S. code) corresponding to the name of the goods in column 79.
Column 9:
Indicate the origin standard of the declared goods enjoying preferential treatment.
(1) If the goods are completely obtained or produced within the territory of one contracting party, fill in: WO.
(two) the goods contain imported ingredients, but are completely produced by materials or parts that have obtained the qualification of origin. Fill in: WP.
(3) Fill in "PSR" if the product meets the specific origin standard, and indicate the percentage that meets the requirements of regional value composition. For example, "PSR" is 60%, which should be higher than the percentage required by the 6-digit HS code in the China-Peru Free Trade Area product-specific rules of origin table.

The tenth column:
Indicate the unit of measurement for the quantity of goods. The unit of measurement should be based on the sales unit listed in the invoice. Such as: kg, Taiwan, parts, pairs, sets, etc.
The commodity number in the sixth column, the product name in the seventh column, the H.S. code in the eighth column, the standard of origin in the ninth column and the unit of measurement in the tenth column should correspond to each other.
The eleventh column:
Indicate the invoice number and date.
The twelfth column:
Fill in the value of the goods.
Column 13:
Fill in the date of declaration, and the applicant shall sign and affix the official seal.
The declaration date should be the actual declaration date.
Box 14: Fill in the date of signing, which shall be signed by the authorized visa officer, and the visa institution shall affix the seal of the visa institution, and indicate the word "Form A" for filing.
The date of review should be the date of certificate entry or the actual date of issuance.
Five, the certificate application time
The application should be made before or on the day of export.
Change of intransitive verb certificate
After the certificate is issued, if the exporter finds that the certificate is wrong, he can apply for changing the certificate. Certificate change must provide the original certificate, and the old and new certificates should be the same batch of goods. The application date and examination date of the new certificate can be consistent with the original certificate. The old certificate is invalid and put on record.
Seven, the certificate is lost
If the certificate of origin is stolen, lost or damaged, the exporter or manufacturer may issue an approved copy of the certificate of origin if he is sure that the original certificate of origin previously issued has not been used, and mark "certified true copy of the original certificate of origin dated _ _ _" on the reissued certificate. The original certificate, relevant bills of lading and customs declaration forms should be provided when the visa is issued.
Eight, the validity of the certificate
It is valid for one year from the date of issuance of the certificate.
Nine, direct flight rules
The transportation of products must conform to the direct transportation rules of the rules of origin of China-Peru Free Trade Agreement.
X. exporter's declaration
If the total customs value of the goods does not exceed US$ 600 or the equivalent of the importer's currency, there is no need to apply for a certificate of origin, and the exporter can make a written statement.
