The contents of the certificate include teaching years, teaching disciplines, class hours, grades, specific classes and years as class teachers, school evaluation opinions and performance certification materials, which shall be signed and sealed by the principal of the school.

Senior teachers who apply for evaluation need to submit the teaching schedule of the school in the past five years (curriculum, work schedule, etc.). , 1 sheet per year and verified and stamped by the academic affairs office of the school, using A4 paper uniformly) as the proof of teaching workload. At the same time, urban primary and secondary school teachers who apply for the evaluation of senior teachers must also submit the original certificate 1 copy of teaching in weak schools or rural schools issued by the education administrative department.

If the applicant is a school leader (principal, vice principal and deputy secretary), truthfully fill in the administrative position, and the county (city, district) Education Bureau will submit 1 assessment opinions on its management ability, level and work performance (achievements). Other middle-level cadres in the school should also truthfully fill in their administrative positions, and the school where they are located will issue 1 evaluation opinions on management ability, level and work performance (political achievements).

5. All localities and schools can submit the pre-evaluation publicity of all applicants in their own places and schools. During the publicity period, all application materials that have been verified to be fraudulent and other illegal acts shall not be submitted and handled according to relevant regulations; If the report is difficult to verify, it shall be truthfully stated and submitted in advance, but the verification shall not be stopped. The verification results shall be submitted to the daily work department of the jury in a timely manner.

7. The applicant's certification materials shall be bound separately with the cover pages "certification materials" (see annex) and "performance materials" (see annex). No certification materials verified and stamped by the unit to confirm the authenticity, and education, experience (qualification), achievements, papers and evaluation opinions without corresponding certification materials are regarded as invalid materials.
