(1) reporting conditions.

1. To apply for and review the deputy senior technical qualification of health professionals, the following conditions shall be met at the same time:

(1) Abide by national laws and regulations, and have good professional ethics and professionalism (those who have been responsible for medical accidents for less than three years, those who have been responsible for medical errors for less than one year, and those who have been subject to administrative sanctions shall not declare during the punishment period);

(two) to participate in the health professional associate senior technical qualification examination and pass the examination, within the validity period, and the time of being employed in the next professional technical position meets the requirements;

(3) The foreign language level of professional titles and computer application ability meet the unified requirements of our province;

(4) Having passed (qualified) or above the annual assessment in the past three years since assuming the present position;

(5) Papers and scientific research meet the requirements (see Annex 1).

2. To declare and review the senior technical qualification of health specialty, the following conditions must be met at the same time:

(1) Abide by national laws and regulations, and have good professional ethics and professionalism (those who have been responsible for medical accidents for less than three years, those who have been responsible for medical errors for less than one year, and those who have been subject to administrative sanctions shall not declare during the disciplinary period).

(2) Bachelor degree or above in medical university, served as deputy chief physician (medicine, nursing and technology) for more than 5 years.

Ordinary college graduates are treated according to the prescribed academic qualifications when they are promoted to senior positions.

(3) The foreign language level of professional titles and computer application ability meet the unified requirements of our province.

(4) Since assuming the present position, the annual assessment in the past three years has been qualified (competent) and above. (5) Papers and scientific research meet the requirements (see Annex 1).

3. Persons who meet one of the following conditions can make an exception to declare and review the senior professional and technical qualifications of health:

(1) conforms to the "Recommended Conditions for Professional and Technical Qualifications of Sichuan Five-Series Exceptional Evaluation (Trial)" (No.1992);

(2) a person who has obtained the qualification of deputy chief physician (medicine, nursing and technology) and served as deputy chief physician (medicine, nursing and technology) for more than 3 years, and won one of the following honors during his tenure:

(1) won the title of model worker at or above the provincial level;

(2) National "May 1st" Labor Medal Winner;

③ Reserve candidates for provincial academic and technical leaders.

(2) All materials are in uniform specifications of а 4 paper size and printed on both sides. The Evaluation Form for Professional and Technical Positions adopts the standard form format uniformly printed by the Provincial Vocational Reform Office. The seals of all materials must be red.
