2. Information required for the trial operation of the environmental protection facilities of the construction project

A copy of the approved environmental impact report (or report form);

The original design drawings and design instructions for environmental protection facilities (required to be stamped with the official seal of the unit with design qualifications and Design seal, drawing reviewer must sign personally);

Proposal filing opinions;

Copy of environmental protection facility construction contract;

3. Construction project The following materials must be submitted for completion acceptance:

A copy of the approved environmental impact report (or report form);

The original design drawings and design specifications of environmental protection facilities (required to add Stamped with the official seal and special design seal of the unit with design qualifications, the drawing reviewer must personally sign);

Opinions on plan filing and approval of trial operation;

Copy of the environmental protection facility construction contract

A copy of the environmental protection facility acceptance monitoring report issued by the monitoring department;

A copy of the original business license of the enterprise;

"Environmental Protection Acceptance of Completion of Construction Projects" Application Report" (report item) or "Construction Project Completion Environmental Protection Acceptance Application Form" (report form item) in quadruplicate, stamped with the branch's opinions and official seal.
