1. This form is only applicable to the change registration of branches;
2. The branch shall fill in the name and affix the official seal on the cover position of this form;
3. 1 Page "Original Approved Registration Items" should be filled in according to the actual situation of the last branch registration, and cannot be blank; "Application for change of registered items" only fill in the changed contents, and other unchanged items can be left blank; "Opinions of relevant departments" column: if the business scope applied by the branch involves the provisions of national laws and administrative regulations and needs to be submitted for approval, the relevant departments should fill in the approval opinions in this column and affix the official seal or special seal for approval of the relevant departments; At the same time, the legal representative of the company should sign his name in the column of "Signature of Legal Representative";
4. The contents of the documents and certificates submitted for applying for registration of change of branches are consistent with the requirements for filling in the documents and certificates submitted for registration of establishment of branches in the Application for Registration of Establishment of Branches.
5. The filling requirements on page 3 are basically the same as those on page 7 of the Application for Registration of Branch Establishment, and the "licensee" can be the person in charge of the branch or the applicant; "Filing of change registration documents and certificates" should be filled in by archivists, which generally means "archival materials have been filed".
Two. Letter of appointment (power of attorney)
Power of attorney (applicable to change registration of branch) The applicant refers to the agent entrusted by the company and the natural person who applies for change registration of branch on behalf of the company. This person should be an employee of the branch office. An agent of a qualified enterprise registration agency may also be an applicant of a branch.
1. The applicant should fill in his name in the blank of "I hereby appoint (entrust) comrades ……", and at the same time, he should write D in the blank of "Responsible for handling articles", select the word "change" in D and mark "V";
2 "Designated (entrusted) time limit" refers to the time limit for entrusting the applicant to carry out the whole process of branch change registration, which should generally be more than half a year;
3. The entrusting (designating) person, that is, the applicant, shall sign his own name at the "Signature of the entrusting (designating) person";
4. The principal refers to the branch company that applies for change registration. "Signature (Seal) of the Principal" shall be signed by the legal representative of the branch and stamped with the official seal of the branch.
5. In the box on the back of this instrument:
(1) If the customer is an employee of the company or branch, paste a copy of his ID card;
(2) If the principal is an agent with agent qualification, paste the agent qualification certificate and a copy of the ID card.
Three. Certificate of business place of branch company
1. This form is applicable to the industrial and commercial registration and change registration of enterprise legal person (business) (including joint-stock cooperative enterprises);
2. The domicile refers to the location of the main office of an enterprise as a legal person, and is the legal registered address of the enterprise as a legal person; The business place refers to the place where an enterprise as a legal person engages in production, business operation or service activities. At this stage, business premises are generally not approved;
3 "domicile (business premises)" column should fill in the specific house number of the district (county) and street (township). If there is no house number, the location from the main reference object shall be indicated;
4. In the column of "Certificate of Property Right Unit", "The property right of this property belongs to all, and it is agreed to provide M to the enterprise in some way for a period of years." The first blank should fill in the full name of the property right unit or the name of the property right owner; The second space is the area of residence (business premises); The third space can be filled with free, rental, etc. According to the actual situation of the enterprise; The fourth space should fill in the name of the enterprise applying for registration; The fifth blank period should be greater than 1 year; The person in charge of the property right unit shall personally sign at the "signature of the person in charge" in this column, and the property right unit shall affix the official seal at the "property right unit (official seal)" and fill in the clear date; If the property right is privately owned, the owner of the property right shall personally sign this column; And should provide a copy of the real estate license stamped with the official seal of the property right unit;
5. If the property right unit has no real estate license, it should be explained by the superior of the property right unit in the column of "Other circumstances that need to be proved", which can generally be filled in as follows: "The property right of this property belongs to XXXX, and the property right certificate has not been (or is being) processed." And affix the official seal of the superior department of the property right unit.
Four. Resume of the person in charge of the branch
The person in charge of the branch shall truthfully fill in the form and sign in the "signature of the person in charge" column. At the same time, post a copy of my ID card and a one-inch bareheaded photo in the corresponding position, and the place where the photo is posted should also be stamped with the official seal of the company.
