If the name on the household registration book is inconsistent with the name on the birth certificate, the name on the birth certificate shall be added in the "previous name" column of the household registration book;

5. If the date and name of birth on the vaccination certificate are inconsistent with the household registration book and the birth medical certificate, you can go to the hospital where the vaccination certificate was originally issued and affix the official seal, or you can reprint a vaccination certificate and affix the official seal;

6. If the date of birth in the household registration book is inconsistent with the date of birth on the birth certificate, you can go to the police station where the household registration is located with the household registration book and the child's birth medical certificate, and go through the formalities of correcting the household registration book information according to the tips of the staff.
